This number is the sum of employees (staff on facility payroll), licensed independent practitioners (physicians, advanced practice nurses, & physician assistants) and adult students/trainees & volunteers.

All HCP except other contract personnel

All HCP

This number is the sum of employees (staff on facility payroll), licensed independent practitioners (physicians, advanced practice nurses, & physician assistants), adult students/trainees & volunteers, and other contract personnel.

All HCP including other contract personnel

Employee HCP (Staff on facility payroll)

Defined as all persons receiving a direct paycheck from the healthcare facility (i.e., on the facility’s payroll), regardless of clinical responsibility or patient contact

Anyone being paid by the facility

Non-Employee HCP (Licensed independent practitioners: Physicians, advanced practice nurses, & physician assistants)

Defined as physicians (MD, DO); advanced practice nurses; and physician assistants only who are affiliated with the healthcare facility, but are not directly employed by it (i.e., they do not receive a paycheck from the facility), regardless of clinical responsibility or patient contact. Post-residency fellows are also included in this\ category.

MD, DO, dvanced practice nurses; and physician assistants who are NOT paid by the facility regardless of clinical responsibility or patient contact. 

Non-Employee HCP (Adult Students/trainees & volunteers)

Defined as medical, nursing, or other health professional students, interns, medical residents, or volunteers aged 18 or older that are affiliated with the healthcare facility, but are not directly employed by it (i.e., they do not receive a paycheck from the facility), regardless of clinical responsibility or patient contact.

Medical, nursing, or other health professional students, interns, medical residents, or volunteers that are NOT paid by the facility regardless of clinical responsibility or patient contact. 

2.2 Any completed COVID-19 vaccine series

Required. Enter the number of HCP (cumulative to date) who completed any primary COVID-19 vaccine series (dose 1 and dose 2 of COVID-19 vaccines requiring two doses for completion or one dose of COVID-19 vaccine requiring only one dose for completion) at the facility or elsewhere (for example, a pharmacy).

This is the total of HCP who have completed a series of COVID-19 Vaccine.  

Question #3

Cumulative number of HCP in question #1, with other conditions:
(IE: conditions other than fully or partially vaccinated)

Given that not all visitors are vaccinated, when someone comes in and reports that they are not vaccinated, then they are sorted into one of the following categories based on additional information

3.1. Medical contraindication to COVID-19 vaccine

Of the HCP in question #1, enter the total number of HCP not receiving vaccination due to either a medical contraindication or exclusion to one or more COVID-19 vaccine(s). Medical contraindications include severe allergic reaction (e.g., anaphylaxis) after a previous dose or to a component of the COVID-19 vaccine. For the purpose of NHSN COVID-19 vaccination surveillance, philosophical, religious, or other reasons for declining COVID-19 vaccine not listed in the Interim Clinical Considerations for Use of
COVID-19 Vaccines Currently Approved or Authorized in the United States are not considered medical contraindications for COVID-19 vaccination; instead, count these under question 3.2 (“Offered but declined COVID-19 vaccine”).

When HCP are not vaccinated due to medical contraindication or exclusion, then they are sorted here

3.2. Offered but declined COVID-19
vaccine

Enter the total number of HCP in question #1 that were offered COVID-19 vaccination but declined.
The following HCP should be counted in this category:

  • HCP declining vaccination because of health conditions that are not considered acceptable medical contraindications to the COVID-19 vaccine.

  • HCP declining vaccination because of religious or philosophical

objection.

  • HCP declining vaccination for reasons other than an acceptable medical contraindication to the COVID-19 vaccine, or those who did not provide any information about the reason why they declined.

When HCP decline the vaccine for any of the following reasons: 

  • Health conditions that are NOT considered acceptable contraindications, 

  • Religious/philosophical objection

  • Other reason, or none given at all

Then they are sorted here

3.3. Unknown COVID-19 vaccination status

Of the HCP in question #1, enter the number of HCP whose COVID-19 vaccination status could not be determined (or who did not meet the criteria for questions #2, #3.1, and #3.2). For example, a facility may not have vaccination documentation for certain HCP.

If a submitted document has not been approved by the time the report is pulled, that individual will be counted as unknown vaccination status. 

Otherwise, when there is no data on the vaccination status of HCP, then they are sorted into this category

Question #4 (HCP receiving Additional
Dose or Booster)
Add all additional doses or boosters of
COVID-19 vaccines HCP received

Required. Enter the Cumulative number of HCP with complete primary series vaccine in Question #2 who have received any booster(s) or additional dose(s) of COVID-19 vaccine at this facility or elsewhere since August 2021. Note: If a facility does not have any individuals who received an additional dose or booster of COVID-19 vaccine, the facility should enter a zero (“0”) for question #4, this will allow the form to save successfully

HCP from question 2 who have received any booster of COVID-19 Vaccine.

5. Up to date with COVID-19 vaccines

Required. Enter cumulative number of HCP in question #2 who are up to date with COVID-19 vaccines according to the definition, found below:

Individuals are considered up to date with their COVID-19 vaccines during the surveillance period of October 3, 2021 through June 26, 2022 for the purpose of NHSN surveillance if they meet (1) of the following criteria*:

  1. An individual received all recommended doses in their primary vaccine series and received at least one booster dose. An individual does not need to receive a second booster dose to be considered up to date at this time.

  2. An individual recently received all recommended doses in the primary vaccine series and is not yet eligible for a booster dose. Individuals who are not yet eligible to receive a booster dose include: a. Those who received their second dose of a 2-dose primary series of an mRNA COVID-19 vaccine (Pfizer-BioNTech or Moderna) less than 5 months ago. b. Those who received a single dose of Janssen less than two months ago.

Any HCP who has received both the primary series, and at least one booster shot UNLESS they are still in the waiting period between primary series and first booster. (Pfizer and Moderna: 5 months, and J&J: 2 months)
